Ballard is a healthcare residence, accredited by the Joint Commission on Healthcare Organizations in Long Term Care and Subacute, which is dedicated to the needs of the residents. As a nationally recognized leader in the field of long term care Ballard has pioneered many innovations in the field. This begins with an organization that strives to improve and adapt services to the ever-changing needs of long-term care residents. Ballard’s ability to continually achieve these results is in part due to the Medical staff associated with the organization. Ballard is truly unique to have an organized Medical staff that are affiliated with area hospitals and work together as a team to insure the quality of the care delivered to residents.

Ballard has been nationally recognized for its innovations in providing spacious therapeutic surroundings that have been planned with the needs of the residents in mind.  Supporting ones ability to maintain the highest level of independence and dignity in a fresh and stimulating environment is our goal.

Spiritual
The fulfillment of the spirit is essential for the residents of Ballard.  We strive to offer ongoing services for all denominations.  Weekly programs are offered in conjunction with local religious groups.  This maintains a connection with congregation members in our community.

Medical Staff Profile
As an organization that emphasizes coordination of services the medical staff play an essential role in this process.  Under the direction of Steve Wolfman DO, Vice president of Medical Affairs, the medical staff participate in a rigorous process of privileging and credentialing in order to assure that the medical staff possess the requisite training, education, and experience necessary to properly care for the patients in their charge.  The medical staff includes Medical Directors that provide the leadership for the various medical and health programs that are offered.

Case Management
Patients are assigned a case manager that acts as a resource for the patients and families in the coordination of care. This occurs at the time of admission and continues through discharge.  Coordination of a discharge plan and interfacing with community resources to ensure a successful return to a community setting is the hallmark of this program.

Services Available

  • Therapy services that are designed to restore and maintain independent functioning. The Our Town Rehabilitation Center™ is a unique feature that is a simulated environment for training and provides residents with the opportunity to practice activities of daily living in a supervised setting.

  • Recreational services are provided as stimulation for resident activity, in an individual and/or group setting. These include exercise programs, discussion groups, field trips, service projects, religious services, arts and crafts, games, bingo as well as customized programs that are chosen based on historical interests.

  • Meals/Dietary Services are prepared by the in house Chef and staff at Ballard. Freshly baked breads and cakes as well home made entrees that are the hallmark of the Ballard menu. All are meals are adapted to any health restriction and alternative entrees are available for the particular resident. Guest trays are available upon request for a nominal fee.

  • A licensed in-house Social Worker and departmental staff provide Social Services. This department provides a variety of support services including support group sessions for residents and families, assistance with federal and state forms, and information on resources available for community care services.

Visiting Hours
Visiting hours are daily 10:00 a.m. until 8:00 p.m. and we can accommodate visitors at any time upon request. Children and pets are welcomed and encouraged.
